“How Do I Get Started?”

Step 1 Come into WELLAWARE and fill out a member packet. This includes a general info sheet, medications list, an informed consent to sign, and a health screening check list.

Step 2 Depending on the results of your health screening we may need a physician’s consent signed by your physician. This is something we can help you with by faxing the form to your physician.

Step 3 Set up an appointment for your initial fitness evaluation (optional) and/or your orientation (required).
