We suggest that you contact your health insurance company regarding coverage.   It is your responsibility to contact your insurance company regarding coverage or prior authorization for any outpatient Diabetes & Nutrition Counseling services requested by your physician.

Helpful Information
When you call your insurance company, you may be asked to provide the following information:

  1. Diagnosis
  2. Reason for referral
  3. Provider
  4. CPT Code (common CPT codes are G0108 and G0109)

Example: My diagnosis is Type II diabetes, I am calling to check on benefits of receiving diabetes education at Boone Hospital Center on an outpatient basis.  My CPT Code is G0108 (Diabetes Self-Management, Initial Assessment) or G0109 (Diabetes outpatient self-management training services, group session).
